The Pathway to Progress Guide helps you and the parent identify the best training plan based on their child’s needs and family priorities. This tool provides guidance, but your clinical judgment should be used when selecting the final plan.

Encourage honest responses based on their child’s needs and family priorities. Completing all the questions will trigger the results and break down the parent's responses to recommend a plan that fit's the family.
Use the form to the right to record the results and document your thoughts and the parent's additional comments. Submitting this form will email you the information you entered.

There are 30 questions in the assessment so be sure to schedule enough time with the parent so they don't feel pressured or rushed. Encourage them to not "overthink" each question but provide their instinctual response.
